What's the Best Skin Rejuvenation Treatment?

My skin is lets say, 'mature' ;) i'd like skin care experts to suggest best ways to rejuvenate and repair from years of ignoring skin maintenance.

A: Treatment of Sun Damaged Skin

Mature skin is frequently skin that has not received adequate sun protection. Sun exposure leads to wrinkles, deep lines, blotchy pigmentation and skin cancers. Some good options...

1.Use a broad spectrum sunscreen and sun-protective measures every day. The SPF should be greater than 15 (better if greater than 30), and broad-spectrum means screens out UVA and UVB. Consider micronized zinc oxide or titanium dioxide products such as MDSolarSolutions. Waterproof or water resistant is better and reapply every 3 hours, more often if swimming, sweating, towelling off, etc  Wear a wide-brim hat whenever possible, seek shade, get out of the "tan is healthy" mentality. Keep your skin hydrated by drinking plenty of water, avoiding excess alcohol and don't smoke.
2.Use a prescription-strength retinoid every evening (e.g. retin A). This helps stimulate collagen production, softening fine lines and improving the tone and texture of the skin.
3.Consider an antioxidant product like Prevage every morning (before applying sunscreen) or a glycolic acid product.
4.Consider chemical peels or laser procedures. The choice is up to you and your doctor. See a board-certified dermatologist to discuss treatment options. You can go from superficial peels (so called "lunchtime peels") to medium or deep chemical peels.
5.Consider Botox or Dysport to treat the dynamic wrinkles (e.g. around the eyes, around the mouth and between the brows)
6.Consider a soft-tissue filler such as Juvederm or Restylane for deeper lines
7.Choose a board-certified dermatologist or plastic surgeon to perform your procedures and make sure that they have extensive experience in the chemical peel or laser procedure that you will be having done

Excising Porous Skin

I have a patch of orange peel skin on my cheek near my nose. It is about a 2"x1" area. It bothers me a great deal and despite extensive research and experimentation (with peels, lasers, etc) I have not found a solution to the problem. Is it possible to actually just remove the affected area? I would rather have a scar than the orange peel skin.

A: Excision of Skin

As the other doctors have pointed out, the first step is to determine what this patch of different skin represents. Go see a dermatologist to evaluate the skin and have a biopsy done to determine why it is so different from the other skin. Once the dermatologist knows what the problem is, he or she can give you options as to the best treatment for this patch of skin.
