Do Lateral Crural Struts Always Add Width to the Nose? (photo)

I'm considering revision rhinoplasty, and a very seasoned revision expert recommended lateral crural struts to help with my valve collapse & pinching. While I would like to fill out the pinching / creases above my nostrils / shield graft edges and improve my breathing, I don't want my nose to be much wider. I've attached pictures of myself smiling - the left is my current nose, the right would be my 'ideal.' Is this achievable, or will lateral crural struts make my nose wider than this?

A: Lower lateral crural strut grafts It is important in all rhinoplasty surgeries to make sure the nose is both aesthetically pleasing and completely functional. The grafts should help alleviate the complaint of the pinched tip with out widening the tip and should hopefully correct the breathing difficulties. Both of these opinions are based on the limited information provided and a full physical examination would need to be done to properly diagnose the cause or causes of your breathing difficulties.

Alar Base Is Wide and Thick after Primary Surgery. Is a Suture Cinch Method The Right Call?

I had nose surgery before, nasal bones were broken. Alar bases looks wide and thick. My doctor wants the suture cinch to narrow the frontal part. Does it going to affect my upper lip or my smile? What are the risks?

A: Alar Base Is Wide and Thick after Primary Surgery

Cinching of the alar will have a minimal effect in the long term and no effect on the width of nasal base. You will need alar repostioning technique, a technique that I developed, whereby, the alae are reduced and repositioned medially. See the before and after photos.

Is This Synechium Scar Tissue After Revision? Does It Cause Swelling? (photo)

1st surgery was in Oct 2008. I had a silicone L-shaped implant inserted. I first noticed this weird nostril-blocking fleshy-thing sometime after the surgery. I ignored it because I thought it was inside of my nose anyways and no one is going to see it unless they look up my nose. Revision in Nov 2011. Implant replaced with ear cartilage. The fleshy nostril-blocking thing on my right nostril is still there. I found out it was called a synechium. Is this scar tissue? Does it cause swelling? Thx.

A: Scar Tissue After Revision

This type of scar tissues can and do cause obstructive symptom and can be corrected with incision  ( laser vaporization is best ) and silicone sheet splint between cut areas for several days.
