I am a 45 year old, African American female living in New York, and have been getting Restylane injections in the nasolabial folds and lips every 8 months for the past three years. I don't know why, but it seems like my body absorbs the material so quickly that the results start to fade within a few months. I am terribly disappointed with the results after three months and was wondering if Evolence would be a better choice for me? If not Evolence, what would you recommend? I want something long lasting, with minimal bruising and swelling.
If you want to get the same product injected into both areas (which may minimize the cost), then we would suggest sticking with Restylane or Juvederm. We avoid using longer lasting (and non-hyaluronic acid based) fillers in the lips because the injections are not reversible. Evolence is one filler that is not reversible and may have a higher incidence of nodule formation in the lips.
In regard to duration, we find that Restylane and Juvederm will last about 6 months when injected into the laugh lines and about 3-4 months in the lips. This is because of the constant movement of the lips. These durations presume that you have been injected to "full correction", which means that you have enough product injected to start.
What should I do about the Evolence that has floated up and is on the bone under my eye?
There is no way to remove Evolence since it is a collagen based filler. (They hyaluronic acid fillers like Restylane and Perlane can be dissolved but not Evolence). Your best bet is to try to camouflage it by filling in around the bump or augmenting below the fullness so it is not as noticeable.
How is this new filler Evolence different from the HA fillers we know and love, i.e. Restylane or Juvederm?
Unlike Restylane and Juvederm which are made from hyaluronic acid, Evolence is made from collagen and that collagen is derived from pigs. Pig (porcine) collagen has been used for many, many years as an injectable filler as the collagen found in pigs is very similar to that of humans. That said, some people prefer not to use an animal based product for religious (muslim or jewish) or ideological (vegetarian or animal-rights) reasons.
The advantage to Evolence is that the price may be lower and that injection related bruising and swelling are reportedly less than with other fillers. The main disadvantage is that if you get a lump with Evolence, it can take months to dissipate.
Restylane and Juvederm are more costly but if you do get a lump or unevenness, it can be corrected with an enzyme called hyaluronidase with smoothes out or dissolves the hyaluronic acid filler.
I do use all of the above mentioned fillers in my practice, but much less Evolence than Restylane or Juvederm.
