It is very difficult to tell exactly where your breasts sit in relation to the crease. This is what really will determine if you need a lift and how much a lift you will need. I would recommend a implant to gain the upper pole fullness with a minimal lift if needed, perhaps a circumareolar or circumvertical. You may be able to get away with a projecting implant and no lift. I would recommend seeking a board certified plastic surgeon to go over your options.
This is a repost w/updated pic. I am scheduled for revision bl on 10/20. My breast are not similar in size or shape. My surgeon agrees that I need a revision & has decided that my skin on the right side needs to be tightened to make the breasts more even. He also said he would lift both sides a little more, using an anchor incision on both sides this time. I have diff implants 210cc &300 & am worried that by having 2 diff implants my breasts will never look similar.

I typically try to perform the same type of lift if I have to do a lift on both sides. This is because each lift gives a different look to the breast. As far as implant sizes go, there are occasions that different size implants need to be used. However if I am doing a lift they I try to remove some breast tissue so that I can try to place similar sized implants bilaterally.

I have moderate sag on my right larger breast and minimum to moderate sag on my left slightly smaller breast. I want to keep the size of the right side. I am currently a 34C/32D and do not want to be larger.
There are many ways to fix asymmetry. In what you have pointed out, yes a lift and implant can be placed to even you out and not add too much volume. If we had some photos it would be easier to say what kind of lift you need. WIthout photos and no measure impossible to say if you need a lift around the areola or a vertical or a full lift.
