How do you Remove Curve Under Nose? (photo)

Is that curve under the nose bone or cartilage? A pre-op doctor told me that no bone (nasal spine I suppose) needs to be removed... But how do you get rid of this? I don't like that part of my nose and don't see how my nose could look good without some of this bone being removed. I attached an image. Also, would removing that bone make my nose too wide and cause nostrils to maybe need to be removed?

A: Excessive anterior nasal spine, maybe

The area of concern you point to in your photo is the region of the anterior nasal spine.  Although a portion of that bone can be taken down slightly, there is another issue that relates to the shape of the skin envelope and how it would redrape into a more concave shape.  This is a little less predictable.  You appear to have an overprojecting nasal tip.  Decreasing the length of projection will involve excising a portion of the anterior nasal spine as mentioned and a portion of your caudal septum.  You may or may not need to combine this with some tip work to help soften the overall appearance.  Photos from the front and worm's eye views would help understand the anatomy of your problem better.

Looking For Second Opinion- Do I Need A Rhinoplasty? (photo)

I visited a plastic surgeon and he acted like I should not do rhinoplasty because he said it would only be minor changes and he did not think it was worth it. What do you think? Do you think I need rhinoplasty? Mainly I do not like the front view of my nose. It is too large and round at the tip and my nose is not symmetrical. My plastic surgeon suggested blepharoplasty instead. Thanks.

A: Rhinoplasty for minor changes

Judging by your photos, a small amount of nasal tip refinement and smoothing of the top of your nose may give you a nice result.  All interventions carry a risk of an unfavorable result not to mention undergoing the discomfort of surgery, so think about these issues carefully before making a decision.  If your plastic surgeon does not feel comfortable with doing your rhinoplasty, then a second opinion may be worthwhile for you, especially if you are currently unhappy with your current appearance.

Is It Possible to Thin the Skin with a Closed Rhinoplasty?

As far as i know the reason for a bulbous tip are thick skin, and large cartilages. Now my question is, is it possible to thin the skin with a closet rhinoplasty? to achieve adequate drapping over the refined structure? And is thinning of the skin necessary? cause i heard its dangerous? Thanks Lidia

A: Thinning skin difficult to do with closed rhinoplasty

Pictures would do wonders in giving you a recommendation on this.  But I think if you have a truly sebaceous or thick skin envelope and you're looking for a finer tip, then I don't know of a better way to thin the skin envelope safely and thoroughly without an open rhinoplasty approach.  Thinning the skin can be dangerous, and if you don't have good visualization, then you can develop a small hole or, even worse, sloughing of a larger area of skin on your nose.  However, this technique is crucial in people with thick skin envelopes to allowing the refined tip to emerge.  Even with thinning, there may still be a chance that the tip may not appear as refined as you would like.
