A doctor told me that tightening the SMAS causes the cheek to flatten. Is this true and if so how do you avoid that? Also, if you just do a composite necklift which address the lower third of the face is the SMAS involved?
No, SMAS should not cause the cheeks to flatten. But keep in mind that the cheeks are not addressed with lifting during SMAS. If your cheeks need to be rejuvenated, that will require add volume to them with fat, Sculptra or fillers.
I was curious because I have been hearing about them recently and wanted to know if it would work better.
A SMAS facelift involves the deeper tissue layer and therefore provides longer lasting and more natural results. A regular facelift surgery only lifts the skin.
I had a SMAS facelift/ neck lift with platysma band plication and liposuction under my chin 15 months ago. As early as 6 months after surgery, my neck bands and saggy neck have returned, there are creases under my chin, pouches on the sides and jowels. My surgeon agrees that I need a revision. My concern is that I am having doubts about the surgeon's skillset. Starting with a new surgeon is financially unrealistic, as this surgery was expensive. I am so disappointed with the results.
All experienced surgeons occasionally have to make minor revisions after facelift surgery. It sounds like your surgeon agrees that a revision is necessary and is willing to work with you. If you have doubts about him/her, you should get second opinions.
