Lower Eyelid Drooping After Blepharoplasty, How Long Should I Wait to Fix it?

I got a blepharoplasty three weeks ago and one of my lower eyelids is drooping. I am 58 yrs old and in good health. I am putting ointment in my eye and taping it into place every night and massaging it several times per day. After the first week, the eyelid came up a bit but no progress has been made in the last two weeks. My surgeon says she can put a stitch in the corner of my eye to pull the lid up if it does not improve. Is this the best way to fix it? How long should i wait?

A: Droopy eyed after blepharoplasties You may have swelling that has pulled the eyelid away from the globe (eye ball). Usually this resolves with time and conservative measures (like you are doing) over a few weeks, but can take a few months. Placement of a tarsorrhaphy stitch to help close the lateral part of the eyelid opening will help for the short term. If this persists more than 3 months or begins to cause symptoms with the affected eye, then surgical intervention may be indicated sooner. You should discuss this with your surgeon.

Swelling - Ice or Heat for Fat Transfer/Bleph?

I am 5 days out of an upper/lower blepharoplasty and full face fat transfer. I am very swollen in my cheeks and my eyes are black/blue and yellow. I thought the swelling would start to decrease by now, but my face is still pretty huge. Can I still do ice? Can I do a warm rice pack? Both would soothe and feel great, I think... not sure if either would harm or hurt.

A: Tear trough problem post blepharoplasty You can try a temporary filler like a Hyaluronic acid product, but this would require maintenance every six months or so. You would likely get a better long lasting result with micro-fat grafting to the tear trough. These grafts should be placed perpendicular to the tear trough and not along the long axis of the tear trough to try to prevent "sausage-like" appearance. Fat grafting has been to shown to improve the quality of the overlying skin as well as thicken the overlying skin which may be further benefit to you. Also, you can try bleaching agents like 4% Hydroquinone to try to lighten the pigmentation you report, but this may provide only another temporary solution.

Considering an Eye Lift, Worried About Potential Scarring.

I have sunken dark circles & wrinkles under my eyes. I am 36 & have had these since my early teens at least. I would like to have an eye lift. I have Melasma, & have found that after acne has healed it is often replaced with a new melasma patch. I am concerned that the scarring will cause melasma under my eyes. I have also had non malignant melanomas removed about 10 years ago, & ended up with keloid scarring for 12 of the 13 removed. Every stitch mark is still visable. Am I likely to scar?

There are many types of skin and we classify these according to your genetic background and pigmentation.  The classification actually takes into account your response to sun damage and scars.  Interestingly, the eyelids are one area that is very resistant to scarring and pigmentation, so that is very much in your favor.  With careful skin preparation using topical agents prior to surgery, and mindful planning of your incisions, you will be able to achieve your goals.
