A: Perlane Lumps

Perlane is a great filler material.  Depending on the area and how long it has been since your injection, these lumps can be very normal.  I would ask you to massage the area gently and be patient.  If it has been more than two weeks since your injection, certainly a call to your physician would be reasonable.  There are always means to reduce lumps if they persist that literally take 10 minutes to accomplish, so please don't assume that nothing can be done.  Enjoy the final results!

Could Vibration Speed Up the Process of Perlane Breakdown?

I had perlane injections in the cheeks, tear trough and naso labial folds one week ago. The eyes and cheeks areas look great, but there is slightly too much filler in the nasolabial folds -- I want that area to flatten out. Will it help to massage vigorously daily or could this cause/proswelling? Is Perlane still malable 7 days? Could applying vibration to the area hasten the breakdown of the perlane. Could repeatedly applying strong pressure to the area help flatten it out?

A: Excess perlane, juvederm, restylane

You can certainly diminish the amount of hyaluronic acid fillers very easily so don't panic.  If you feel that you have an excess in any area, I would recommend conservative observation only for the first 2 weeks.  This allows any swelling to completely resolve.  Gentle massage is all that is necessary.  Certainly don't cause yourself any discomfort in the massage.

Hyaluronidase is a widely available commercial enzyme that will dissolve the filler within minutes.  This is off label but very safe and successful in my experience.  I have used this on patients over the last 10 years with immediate satisfactory results.  Consider this if your filler is excessive after 2 weeks.  When used appropriately, the surgeon can safely remove only the excess leaving a quality result.

Creases and Bulges After Perlane Injections

I had Perlane injected by end of December 2008 for my nasolabial folds. I developed trangular-shaped bulges on my cheeks, which also caused creases along my cheekbones. I looked pudgy and tired! The surgeon tried to fill lines first, but they turned blue. Yesterday, he injected Vitrase to remove it all. It looked a lot better a few hours later, but this morning, it looked even worse! What is going on? Am I now doomed to have these creases from the chubby triangles forever?

A: Post Perlane Problems

Perlane is a great filler for deep nasolabial folds.  It does stay soft for a couple of hours after injection and can be molded somewhat during that time.  Movement such as smiling might have contributed to the bumps, it is hard to second guess your surgeon, however.  Hyaluronidase is the right move when filler needs to be removed.  It works within 48 hours.  It has been reported to cause an allergic reaction in a small number of people.  It is possible that you are one of them.  This reaction will subside, and you will be as good as new, of course you will still have your original problem.  Perhaps a softer filler like Juvederm would "sit" better in you.
