Im curious as to whether alarplasty can be done under local anasthesia? Can any rhinoplasty be done this way?
I would say that most patients will tolerate reduction alar surgeries under local. By that I refer to techniques designed to reduce the size of the tip, refine the tip, reduce nostril size etc. Weir incisions and alar base reductions are commonly completed under a local anesthetic in an operative suite. Composite grafts or other surgeries to increase the tip size, projection, etc., are usually best performed under a general for the comfort and safety of the patient. Great question.
Your help and expert input will be appreciated, as this has become a huge concern after my primary rhinoplasty. I am now >6 months post surgery, and I continue to have bruising around my eyes particularly on the upper eyelids, this is not isolated to one side. Furthermore, after surgery, I notice that my eyes have a very sunken look to them, almost a sagging beneath the orbital with some bruising. Also, my eyes tend to feel and get tired often.
Generally speaking, bruising should clear within 2 weeks or so after rhinoplasty. It sounds as though you may have hemosiderosis from prolonged bleeding or stasis (blood that collects in a pool under the surface). It is more commonly an issue in areas that have poor circulation. The same can cause localized swelling. I would recommend gentle massage, warm compresses and vitamin K cream. Lasers directed at pigment have been very successful as well.
Hello there, i'm contemplating having rhinoplasty. By looking at these pictures, what do you recommend? From a professional opinion, is it needed or not.
I rhinoplasty would enhance your face and bring out your other nice features (eyes, cheek bones, jawline...) I would recommend lowering the dorsum, narrowing the nasal bones and refining the tip with excision and suture contouring techniques. In my hands, I can achieve this best with an "open" approach. Hope that helps.
