Hypertrophy is characterized by enlargement of an organ or tissue resulting from an increase in the size of its cells. A hypertrophic scar is one that has become enlarged in such a way.
Hypertrophic scar causes
While the exact causes of unfavorable scars are not yet perfectly understood, it is clear that anything that increases the length or vigor of the inflammatory response that normally occurs with healing will increase the risk of such a scar. Poor or delayed healing after surgery is one possible cause of hypertrophic scarring.
Hypertrophic scar treatment
It is best to initiate hypertrophic scar treatment as early as possible during the healing process. If a scar is older than two years, it is unlikely that anything short of surgical removal or scar revision will work. Basically, scars should be injected as soon as possible with kenalog or steroids. This should be followed with silastic or silicone gel sheeting. Other interventions that may help include the following:
- Pressure
- Massage
- Topical paper tape therapy
- Topical or intralesional steroids
- Topical silicone gel preparations or sheeting
- Pulsed dye laser treatment or IPL
- Aldara cream (off-label)
Sometimes hypertrophic scar treatment can take many years to be successful.
Hypertrophic scar prevention
If you know that you are a poor healer and prone to hypertrophic scars, you should always notify your surgeon or dermatologist first so that if you sustain another injury somewhere else or require surgery, then further precautions can be taken to help minimize the appearance of your scar. If possible always avoid surgery in visible areas.
